A Pentagon investigation, according to a Bloomberg report, found that flawed intelligence, outdated imagery and overreliance on AI-assisted targeting contributed to the U.S. strike on the Shajareh Tayyebeh school in Minab, Iran. The February 28 attack killed more than 150 people, including at least 123 children, according to officials cited in the report.
